Incident report

This form is used to record altercations with patrons, violations of library policies, and other occurrences with patrons that you might feel other staff should know. These reports will be recorded for all staff to see on the incident log. Please record the information in as much detail as possible.

Examples of items to record on this form might be, but are not limited to:

Belligerent behavior or yelling;
Viewing pornographic material;
Violent behavior;
Having to speak to the same patron multiple times about a minor policy infraction (e.g. speaking too loudly).

Please do not use this form to submit patron or staff accident reports. Use the patron accident report or staff accident report for those.
